Medical Social Worker Position at Saint Francis Hospital and Medical Center:

Position is for a full time temporary medical social work position at Saint Francis Hospital and Medical Center's Congestive Heart Failure outpatient clinic.  Requires Master's degree from a CSWE accredited school of Social Work; knowledge and abilities essential to the successful performance of the duties assigned to the position; demonstrates resourcefulness, effective written and oral communication, diplomacy, organizational and analytic skills; and self-directed, assertive and creative in problem solving, systems planning and patient care management in a high volume work environment. Three years' experience in an acute care hospital setting is preferred.  Will conduct in-depth psychosocial assessment, provide patients with supportive counseling, advocacy and care coordination and will coordinate/facilitate support groups and education sessions.  Experience working with LVAD/Heart transplant and chronic illness is highly preferred.

We currently have an opportunity for a Social Worker /Research Assistant Professor in Transplantation to a join our team at our Chicago, IL location.

The Social Worker/Research Assistant Professor in Transplantation is an advanced practice clinician, educator, investigator, and role model for the social work profession, to NMH and the professional community. This position is responsible for complex psychosocial interventions and facilitating the plan of care for potential and actual living donors (IDA role) or potential and actual recipients and their families from initial contact through post-donation or transplantation.

The selected individual will also hold a faculty appointment at Northwestern University and must meet the university's faculty standards as well as support the mission, vision, and values of Northwestern University. Additional responsibilities will include collaborating with other departments and integrating the department's functions and services with other aspects of the patient care process; developing observable and measurable goals and expected outcomes in the areas of problem resolution, utilization and resource management, and patient satisfaction; and initiating and participating in scholarly inquiry regarding living donors or recipients as appropriate.

The Social Worker /Research Assistant Professor in Transplantation may be placed in one of two primary functions: The Transplant Generalist role or Independent Donor Advocate (IDA) role.

Transplant Generalist: The Transplant Generalist meets directly with patient/family to perform a comprehensive assessment including social, emotional, cultural, mental status, environmental and financial circumstances in conjunction with interdisciplinary assessment of the patient. Selected candidate will also recommend a plan of intervention based on mutually established goals.

Independent Donor Advocate: The IDA provides social work services to potential and actual living donors and their families as described for the generalist but focuses exclusively on living donors.

Requirements:

  • Master's degree in Social Work from a school of social work accredited by CSWE; a doctoral degree is strongly preferred.
  • State of Illinois Licensure (LCSW)
  • Minimum of five years post master's experience in a health care environment.
  • Three years experience in a medical social work specialty (examples: Women's Health, Oncology, Cardiology, Neurosciences, Dialysis, HIV, Hospice, Transplant, etc.), or with a special patient population within a general medicine service such as sickle cell, cystic fibrosis, rheumatology, trauma, geriatrics, etc. Experience in transplantation strongly preferred.
  • Analytical skills necessary to independently collect, analyze, and interpret data, resolve problems requiring innovative solutions and to negotiate in sensitive situations.
  • Demonstrated research skills as evidenced by published papers in peer-reviewed journals. A strong preference will be given to candidates with experience as a principal or co-principal investigator.
  • Demonstrated skills in grant writing.
  • Minimum of 2 semesters of graduate fieldwork instruction.
  • Demonstrated critical thinking skills and independent clinical practice using consultation as appropriate.
  • Demonstrated leadership skills.
